Before our first meeting, please review and complete my practice documents. The intake questionnaire is included so that I may gather some helpful background information. Some of the questions may feel sensitive and you may not feel prepared to answer them before we meet. Please only answer what you feel comfortable with.

My office is located at 3201 Wilshire Blvd., Suite 202. There is parking on available on the cross street, Franklin St., or on Wilshire. When you enter the suite, there are call lights located on the wall. Please flip mine to let me know you have arrived.
